MAKE A REFERRAL
Anyone may refer a patient to Kaua`i Hospice. Referrals may come when the patient, a friend, caregiver, relative, social worker, nurse, hospital, nursing home, Home Health Care Agency, or the patient's private physician calls Kaua`i Hospice. Our staff is available to visit with the patient and family prior to admission in order to make an assessment and explain the services provided.
